PharmaCompass PK/PD is an advanced antimicrobial dosing optimization application currently in development by Gregory K. Perry, PharmD, BCPS, BCIDP — a board-certified infectious diseases pharmacist with over 30 years of clinical experience in critical care and antimicrobial stewardship.

What is PharmaCompass PK/PD?

PharmaCompass PK/PD is a Monte Carlo simulation-based clinical decision support tool designed to help practicing pharmacists optimize antimicrobial dosing in real time. Unlike traditional dosing calculators, PharmaCompass PK/PD uses probability-based modeling to predict the likelihood that a given dose will achieve the pharmacodynamic target needed to maximize bacterial kill while minimizing toxicity risk.

What It Will Do for the Practicing Pharmacist:

  • Optimize antimicrobial exposures — calculate the probability of pharmacodynamic target attainment (PTA) for individual patients based on their unique renal function, weight, and pathogen MIC
  • Support AUC-guided dosing — particularly for vancomycin and other narrow therapeutic index agents
  • Monte Carlo simulation — run thousands of simulated patient scenarios to determine the optimal dose and interval for your patient population
  • Organism-specific modeling — tailor dosing recommendations based on site of infection, organism, and local resistance patterns
  • Stewardship integration — support de-escalation decisions, IV-to-oral transitions, and duration of therapy recommendations grounded in PK/PD principles
